    The House Committee on Health and Human Resources moves to amend the bill on page one, section four, line twenty three, following the word “anesthesia” by deleting the words “certificate or”;

    On page two, section five, line seventeen, following the word “following” by deleting the words “certificates and/or”;

    On page two, section five, line nineteen, following the number “2” by deleting the word “Certificate” and inserting in lieu thereof the word “Permit”;

    On page two, section five, line nineteen, following the number “2” by deleting the word “Certificate” and inserting in lieu thereof the word “Permit”;

    On page three, section six-a, line eight, following the word “analgesia” by deleting the word “permit” and inserting in lieu thereof the word “use”;

    On page three, section six-a, line ten, following the word “analgesia” by deleting the words “without a permit”;

    On page six, section six-b, line eleven, following the roman numeral “II” by deleting the words “Certificate” and inserting in lieu thereof the word “Permit”;

    On page six, section six-b, line twelve, following the roman numeral “II” by deleting the letter “Certificate” and inserting in lieu thereof the word “Permit”;

    On page eight, section six-b, line fifteen, following the word “a” by deleting the word “certificate” and inserting in lieu thereof the word “permit”;

    On page eight, section six-b, line sixteen, following the roman numeral “II” by deleting the word “Certificate” and inserting in lieu thereof the word “Permit”;

    On page ten, section six-c, line six, following the word “Anesthesia” by deleting the words “Certificate or”;

    On page twenty one, section eight, line eight, following the roman numeral “II” by deleting the word “Certificate” and by inserting in lieu thereof the word “Permit”; and 

    On page twenty-one, section eight, line twelve, following the letter (b) by deleting the entire subsection and replacing in lieu thereof the following:

    “Dentists utilizing a West Virginia licensed dentist anesthesiologist shall have their office inspected to the level of permit held by the dentist anesthesiologist.”
